Description of project aims

To investigate the impact of the introduction of community link workers on rural, remote dwelling individuals, their families and carers, the third sector, and primary care providers. 
To identify barriers and facilitators to the implementation of a community link worker social prescribing programme in rural settings. 
To identify indicators of change for monitoring and measuring the impact of community link worker social prescribing outside urban areas.
